In this full-time role, you will provide much-needed support to seniors and their families through compassionate counseling and resource coordination. Work Environment\n

This position is remote, allowing for flexibility while maintaining regular communication with clients and their families. The work will be conducted largely from your home office, with occasional in-person visits to Hospice Centers as needed. Key Responsibilities\n

• Conduct thorough psychosocial assessments of senior clients, which include evaluating their emotional, mental, and social factors that impact their overall well-being.

• Develop tailored care plans in collaboration with clients and their families, incorporating their specific needs, goals, and preferences for greater engagement in their care.

• Provide counseling to seniors, assisting them in navigating life transitions, such as loss or health changes, and offering emotional support to their families during these challenging times.

• Advocate for clients' rights by connecting them to essential community resources, facilitating access to healthcare services, and ensuring they receive the necessary support.

• Engage with interdisciplinary teams, including nurses and other healthcare professionals, to enhance client care and promote communication among all parties involved.
\n
• Perform regular follow-up visits, either virtually or in person, to assess clients' living situations and well-being, and to address any emerging issues or concerns promptly.

• Facilitate open communication among families involved in a client’s care, helping to resolve conflicts and misunderstandings to ensure cohesive support.
\n
• Educate clients and their families on available resources, support groups, and activities that can foster social engagement and improve their quality of life.
\n
• Maintain detailed and accurate documentation of client assessments, care plans, and progress notes, ensuring compliance with ethical and legal guidelines.

• Stay updated on current social work practices and policies, as well as local community resources, to provide informed and effective support.

Patient Population / Client Focus\n

This role primarily serves the senior population, focusing on those in hospice care or experiencing end-of-life transitions. Clients may present with a range of psychosocial challenges, including grief, mental health disorders, social isolation, and varying degrees of dependency on others for care. The ideal candidate will possess a deep understanding of the complexities of geriatric social work and the ability to address their unique needs empathetically.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current license in the state of New Jersey as a Master Social Worker (LMSW).

• A minimum of two years of relevant experience in social work, particularly within a community center or geriatrics environment.

• Strong foundational knowledge of social work principles, especially in relation to aging-related issues and the resources available for seniors.

• Exceptional assessment, counseling, and conflict-resolution skills.

Compensation and Benefits\n
