Youth Counselor/Transition Specialist- Onsite- Indianapolis, IN Indianapolis, IN, US Salary Range: $18.00 To $21.00 Hourly Job Title: Transition Specialist Location: Indiana Duration: Contract Job Description We are seeking a Transition Specialist to support youth transitioning from juvenile correctional facilities back into their communities. Responsibilities Develop and coordinate youth transition and reentry plans. Teach life skills and support educational goals. Help youth obtain IDs, Social Security cards, birth certificates, and other required documents. Assist with returning to school, employment, apprenticeships, or workforce programs. Coordinate with schools, families, community agencies, and state organizations. Conduct follow-up calls after youth are released. Participate in transition planning meetings and represent the program at community events. Preferred Qualifications Experience in youth services, case management, social work, juvenile justice, corrections, counseling, or community outreach.
