Job Opportunity: Registered Nurse Clinical Support Coordinator
Our organization is looking for a Registered Nurse Clinical Support Coordinator to enhance patient-centered clinical coordination within our acute care facility. This position involves assisting healthcare providers with various tasks, including:
- Conducting assessments
- Developing care plans
- Performing triage
- Facilitating follow-up care
Your efforts will be crucial in ensuring smooth and safe transitions of care for our patients. You will work closely with a multidisciplinary team, providing education to patients and their families while ensuring that electronic medical records (EMR) are accurately maintained.
