Care Coordinator

As the primary point of contact for families across the US, the full-time remote Care Coordinator will manage the logistics of caregiving, develop comprehensive care plans, and provide ongoing support to families navigating complex care needs. Key responsibilities Manage member-facing relationships, ensuring high-quality and timely execution of care priorities Develop and oversee tailored care plans while coordinating medical appointments, medications, and insurance needs Collaborate with Care Advisers to enhance service delivery and support team effectiveness through knowledge sharing Required qualifications Bachelor's degree in Social Work, Public Health, Healthcare Administration, or a related field; advanced degrees are a plus 4-6+ years of healthcare experience in case management, crisis intervention, or coordination Proven ability to navigate the healthcare sector and communicate empathetically and clearly Experience in developing and facilitating care plans tailored to individual needs Strong adaptability to manage up to 40 care projects efficiently while maintaining compassion
