Job Description The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. Daily Responsibilities Oversee two case managers Implementation of safe and appropriate patient discharge plans Staff training and development Utilization Management and Review Financial Case Management (connecting the under-insured/uninsured to financial assistance) Hours for this Position Monday - Friday 8:00am – 5:00pm Advantages of this Opportunity Competitive salary, $70,000-$75,000 negotiable based on relevant experience Benefits offered, Medical, Dental, Vision, and PTO Fun and positive work environment Qualifications 4-5 years Hospital Case Management experience with some type of critical care/medically complex case background within the last 3 years Financial case management experience ( connecting the underinsured/uninsured with financial service to assist with coverage) Additional Information Interested in being considered?
