Field-Based RN Case Manager: Medicare Care Transitions

CVS Health is seeking an RN Case Manager in the Richmond, VA area to support Medicare/Medicaid members with complex needs. You will conduct home, hospital, and community visits, develop care plans, coordinate services, and work with interdisciplinary teams to improve outcomes.
