AVIR at Alice is seeking a compassionate and dedicated Per Diem Physical Therapist (PT) to join our in-house rehabilitation team. This is an excellent opportunity for a Physical Therapist who enjoys providing quality, patient-centered care and collaborating with an interdisciplinary team to help residents improve mobility, regain independence, and achieve their functional goals. Position Responsibilities Evaluate residents and develop individualized physical therapy treatment plans. Provide skilled physical therapy services based on residents' needs, goals, and established plans of care. Implement therapeutic exercises, mobility training, balance activities, gait training, transfer training, and functional activities. Monitor residents' progress and modify treatment plans as appropriate. Provide education to residents and caregivers regarding mobility, safety, therapeutic exercises, and functional activities. Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff, physicians, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation. Participate in care planning and communicate residents' progress, needs, and recommendations to appropriate team members. Maintain compliance with facility, state, and federal regulations. Qualifications Current Texas Physical Therapist (PT) license in good standing. Graduate of an accredited Physical Therapy program. Strong clinical, communication, and organizational skills. Experience in skilled nursing, subacute rehabilitation, or a similar setting preferred. Compassionate, professional, and resident-focused approach to care. New graduates are encouraged to apply.
