Intake Coordinator – LCHSA

Position Summary

We are seeking an experienced and highly organized Intake Coordinator to join our rock-star intake team.

The Intake Coordinator is responsible for managing all assigned referrals from initial intake through the start of care, ensuring compliance with company policies, payer requirements, and applicable regulations. Each referral may arrive at a different stage of the intake process, requiring the coordinator to assess the case, identify outstanding requirements, and facilitate a smooth transition to active home care services.

As the first point of contact for referral sources, patients, families, and community partners, the Intake Coordinator represents the agency professionally while providing exceptional customer service. The coordinator works closely with referral sources, managed care plans, discharge planners, physicians, patients, and internal departments to ensure timely and accurate admission to services.

Key Responsibilities

  • Manage and follow up on all assigned referrals through admission and start of care.
  • Verify eligibility, authorizations, and payer requirements for home care services.
  • Coordinate communication between referral sources, patients, caregivers, managed care plans, and agency staff.
  • Obtain and review all required documentation for admission.
  • Ensure compliance with New York State Department of Health regulations and agency policies.
  • Track referral status and maintain accurate documentation in agency systems.
  • Answer incoming calls professionally and courteously, providing information regarding agency services.
  • Work collaboratively with staffing, nursing, and administrative teams to ensure seamless service initiation.
  • Maintain strong relationships with hospitals, rehabilitation facilities, MLTCs, physicians, and community referral sources.
  • Meet intake and admission timelines while maintaining a high level of accuracy and customer service.

Qualifications

  • Previous experience as an Intake Coordinator in a Licensed Home Care Services Agency (LCHSA), Managed Long-Term Care (MLTC), home care agency, or related healthcare setting.
  • Strong understanding of the MLTC enrollment process and Medicaid eligibility requirements (Medicaid applications are not completed in-house).
  • Knowledge of home care authorization processes, payer requirements, and referral management.
  • Excellent follow-up skills with the ability to manage multiple referrals simultaneously.
  • Strong verbal and written communication skills with a professional demeanor.
  • Ability to build and maintain relationships with referral sources and community partners.
  • Detail-oriented with exceptional organizational and problem-solving skills.
  • Proficiency with electronic health records (EHR) systems and Microsoft Office applications.
