Waymark is hiring an Outreach Specialist/Care Coordinator Manager for the Greater Chicago area. You will lead a team of outreach & care coordinators, drive patient outreach and coordination, and collaborate with cross-functional partners to optimize care delivery. You will coach staff, shape CC experience, and support recruitment, training, and deployment while expanding community partnerships and PCP engagement.
