Step into a dynamic Nurse Practitioner role focused on home health risk assessments for Medicare Advantage members in Shoshone County, ID, including areas such as Kellogg, Pinehurst, and Osburn. This flexible per diem position is ideal for clinicians who enjoy variety and autonomy, as youll conduct in-home visits to assess patient health, educate, and support improved care outcomes.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments with each visit averaging 45 minutes
• Evaluate patients prior and current medical history, chronic conditions, and medication lists
• Educate patients on their medical conditions and medication management
• Identify gaps in care and provide actionable recommendations for follow-up, without being responsible for treatment planning or prescriptions
• Complete accurate and timely documentation for each assessment

Qualifications:

• Active Nurse Practitioner license in Idaho
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• At least 6 months of current NP experience is highly preferred
• MSN degree from an accredited nursing program

For each evaluation, youll earn between $80-$90, plus a stipend for mileage and compensation for administrative time.
