Compassionate Hospice RN Case Manager

Gentiva

Gentiva seeks a compassionate Hospice RN / RN Case Manager to lead patient assessments, deliver skilled nursing interventions, and develop personalized care plans in home or community settings. You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure holistic, patient-centered care. CPR-certified, licensed in-state, and 1+ year hospice/home health experience preferred. Join a team that supports patients and families with dignity and comfort, providing education #J-18808-Ljbffr
