What You'll Do

• Provide comprehensive assessments and coordinate patient care using the nursing process
• Develop, implement, and modify individualized Plans of Care in collaboration with patients, families, and healthcare teams
• Supervise Certified Nursing Assistants (CNAs) to ensure quality patient care
• Complete detailed documentation following all federal, state, and agency regulations
• Educate patients and their families on health management and safety
• Ensure a safe and clean environment by following safety protocols and standards
• Maintain high standards of privacy and confidentiality at all times

What You Need

• Required:
  • Valid Registered Nurse (RN) license in Illinois
  • Experience in home health nursing preferred
• Preferred:
  • Knowledge of the nursing process and patient assessment techniques
  • Strong communication and teamwork skills
