What You'll Do

• Evaluate patient physical therapy needs in accordance with physician's orders
• Develop and implement individualized treatment plans to improve mobility and independence
• Provide age-appropriate care for pediatric to geriatric patients
• Communicate effectively with patients, families, physicians, and healthcare staff
• Document all patient care activities accurately and timely in the electronic medical record
• Assist with patient transfers and positioning as needed
• Ensure all treatment goals are met through proper assessment and follow-up

What You Need

Required:

• Valid physical therapy license or eligible for licensure in Illinois
• Prior experience working in a home health setting preferred
• Strong communication and patient interaction skills

Preferred:

• Experience working with a diverse patient population including pediatric and elderly patients
• Ability to work independently and as part of a healthcare team

About the Assignment This position involves providing direct physical therapy services to patients in a home health setting, focusing on improving patient mobility and functional independence. You will work closely with a multidisciplinary team and engage with patients across a range of ages, ensuring they receive personalized, effective care in their home environments.
