Doctor of Pharmacy (PharmD) degree .\nActive Pharmacist license in South Carolina\nPrevious inpatient experience is preferred but clinical inpatient rotations experience or 1 year of residency will be accepted\nIf no inpatient experience, retail management experience will be considered.\nEPIC experience is preferred.

Job Description

The Adult Inpatient Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to hospitalized adult patients. This role collaborates with physicians, nurses, and other healthcare professionals to optimize medication therapy, improve patient outcomes, promote medication safety, and ensure compliance with regulatory and organizational standards. The pharmacist serves as a clinical resource for medication management, participates in interdisciplinary rounds, and supports quality improvement initiatives throughout the hospital.\n \nKey Responsibilities\nClinical Pharmacy Services\nReview, verify, and monitor medication orders for accuracy, safety, and therapeutic appropriateness.\nAssess and optimize pharmacotherapy for adult acute care patients.\nProvide dosing recommendations for antibiotics, anticoagulants, renal-adjusted medications, and other high-risk therapies.\nPerform pharmacokinetic monitoring and therapeutic drug management.\nEvaluate patients for medication-related problems and recommend evidence-based interventions.\nParticipate in interdisciplinary patient care rounds and collaborate with providers regarding treatment plans.\nMedication Safety & Operations\nVerify medication orders and ensure compliance with hospital policies and regulatory requirements.\nSupport medication-use safety initiatives and adverse drug event prevention.\nAssist with medication reconciliation upon admission, transfer, and discharge.\nProvide oversight of formulary compliance and therapeutic interchange programs.\nRespond to medication-related inquiries from physicians, nurses, and other healthcare team members.\nPatient Care\nEducate patients and caregivers regarding medications, expected outcomes, side effects, and adherence strategies.\nSupport transitions of care efforts to ensure continuity of medication management across care settings.\nAssist with discharge medication planning and counseling as needed.\nClinical Support & Education\nServe as a drug information resource for healthcare providers.\nEducate pharmacy staff, residents, students, and other clinicians on medication management and best practices.\nParticipate in department meetings, committees, and clinical initiatives.\nQuality Improvement\nParticipate in medication safety, antimicrobial stewardship, and quality improvement programs.\nContribute to development and implementation of clinical protocols, order sets, and evidence-based pharmacy practices.\nSupport regulatory, accreditation, and compliance activities.
