Family Care Coordinator

Positions Available in Licking County

Behavioral Health Care Partners (BHP), formerly Moundbuilders Guidance Center, is seeking Family Care Coordinators to provide care coordination services to youth and their families. This position works with youth from birth through age 22 if still enrolled in school and are experiencing mental health and/or substance use concerns.

Responsibilities

• Provide care coordination and support to youth and families.
• Screen and assess clients and identify needs, strengths, and risk factors.
• Coordinate treatment and services with families, schools, community resources, and internal/external providers.
• Monitor client progress and ensure services are delivered appropriately.
• Meet individually with clients and facilitate groups.
• Maintain required documentation and participate in shared on-call responsibilities.

Qualifications

• Associate's degree in Human Services or related field.
• 1–3 years of case management experience with individuals experiencing mental health and/or substance use concerns.
• QMHS required; LSW or LPC preferred .
• Valid Ohio driver's license.
• BLS/CPR certification required.
• Strong communication, collaboration, and organizational skills.
• Basic computer and documentation skills.
