Hospice Social Worker & Case Manager - On the Road
Moments Hospice
Moments Hospice in Missouri seeks a licensed Social Worker to provide psychosocial services to terminally ill patients and their families on a road-based assignment. You will contact patients within 48 hours of admission and participate in weekly interdisciplinary team meetings. The ideal candidate has a Master’s in Social Work (or a BSWS with healthcare experience), at least two years in healthcare, a valid driver’s license, and state licensure.
