Work Environment\n

This role will operate in a remote setting, primarily associated with a hospice center, allowing for flexibility while effectively engaging with patients and their families. As a social worker in hospice, you will work collaboratively with an interdisciplinary team consisting of physicians, nurses, and other healthcare professionals dedicated to providing high-quality end-of-life care. Your role will involve utilizing various telehealth platforms for patient interactions, ensuring that care can be delivered where it is needed most.

Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to identify mental health, substance use, and medical issues impacting patients and their families.

• Provide behavioral health counseling services tailored to the unique needs of individuals in hospice care, including individual therapy sessions, family counseling, and crisis intervention.

• Create comprehensive treatment plans that address patient-specific needs and engage in ongoing treatment evaluation to monitor progress and outcomes.

• Facilitate suicide risk assessments and ensure appropriate interventions and escalations to medical professionals as necessary.

• Coordinate care with local resources and support networks, providing patients and families with access to medication assistance programs and community services.

• Collaborate with Quality Improvement teams to enhance data collection practices and ensure compliance with best practices in patient documentation.

• Utilize electronic health record (EHR) systems for accurate documentation of patient interactions and therapy sessions to maintain continuity of care.

Qualifications\n

Compensation and Benefits\n
