Job Title: Care Coordinator – Intellectual Disability (ID) Services Company: Independent Home Health Position Type: Full-Time Salary Range: $50,000 – $55,000 per year (Based on case management experience) Location: Luzerne, Wyoming, and Lackawanna Counties Caseload (Reporting to Home Office in Towanda, PA) Reports To: Towanda Office Manager / ID Services Director Job Summary: Independent Home Health is seeking a dedicated, highly organized, and empathetic Care Coordinator to join our team. This position manages a stable caseload of around 45 individuals with intellectual and developmental disabilities residing across Luzerne, Wyoming, and Lackawanna Counties. While day-to-day service coordination occurs in the field and remotely across the multi-county caseload area, the designated home office for reporting, administration, and oversight is located in Towanda. In this role, you will report directly to the Towanda Office Manager / ID Services Director and serve as the primary link between our clients, their families, direct care staff, and county funding systems. Key Responsibilities:

Case Management & Coordination: Oversee a dedicated caseload of around 45 individuals within Luzerne, Wyoming, and Lackawanna Counties, ensuring all aspects of their home health and community-based services are delivered seamlessly under the Independent Home Health name. Staff Coordination & Case Matching: Actively match clients with appropriate direct care staff based on client needs, staff skill sets, personality compatibility, and scheduling availability to ensure consistent, high-quality care. Plan Implementation & Monitoring: Review, implement, and monitor existing Individualized Support Plans (ISPs) in strict compliance with Pennsylvania ODP waiver guidelines (Consolidated, Community Living, or P/FDS waivers).

Note: The creation of the ISPs is handled externally; this role is strictly responsible for coordination, tracking, and execution. Director & Manager Reporting: Work directly under the guidance of the Towanda Office Manager / ID Services Director, submitting regular case evaluations, administrative updates, and compliance reporting to the Towanda home office. Advocacy & Field Monitoring: Coordinate care with regional administrative entities (such as Luzerne-Wyoming Counties MH/DS and Lackawanna County MH/ID/EI), medical professionals, and family members. Conduct regular field visits to monitor quality of care. Compliance & Documentation: Maintain meticulous electronic case records, progress notes, and service authorizations in accordance with Pennsylvania regulations and Independent Home Health agency standards. Required Qualifications (Non-Negotiable):

Professional Experience: Minimum of [1 to 3] years of direct Case Management / Service Coordination experience within the human services or home health sector. Specialized Knowledge: Proven professional experience specifically working with individuals with Intellectual and Developmental Disabilities (ID/DD). Regional Requirements: Must have a valid driver’s license, a reliable vehicle, and a willingness to travel extensively across Luzerne, Wyoming, and Lackawanna Counties for client care, alongside commuting to the Towanda office as scheduled for reporting and supervision. Regulatory Familiarity: Deep understanding of Pennsylvania's Office of Developmental Programs (ODP) systems, Medicaid waiver programs, and person-centered compliance frameworks. Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a closely related field (or an equivalent combination of education and experience verified by state regulations). Preferred Qualifications:

Prior experience working within an independent home care or home health business model. Existing familiarity with the Luzerne, Wyoming, and Lackawanna County community resource networks and local county administrative structures. What We Offer:
