Responsibilities

• Perform initial PT evaluations and develop individualized treatment plans for home health patients.
• Deliver therapeutic interventions to improve mobility, strength, balance, gait, and functional independence in patient homes.
• Reassess progress regularly, adjust treatment plans, and document outcomes in Home Care Home Base and PointCare.
• Educate patients and family caregivers on safe mobility, fall prevention, and home exercise programs.
• Collaborate with the interdisciplinary team to coordinate durable medical equipment, referrals, and discharge planning.
• Ensure safety, infection control, and compliance with regulatory standards during home visits.

Requirements

• Active physical therapy license in Oregon
• Doctor of Physical Therapy (DPT) degree or equivalent
• Home health experience preferred; must have hands-on home care exposure
• Experience with Home Care Home Base and PointCare software
• Valid driver’s license and reliable transportation; willingness to travel to patient homes
• Ability to work effectively in a home health setting and adhere to travel schedule
