This is a fully remote position, which provides the flexibility to work from your home office while serving patients in various locations across California. As a member of our healthcare team, you will collaborate closely with colleagues from diverse disciplines, including medical doctors, psychologists, and other social workers. Key Responsibilities\n

• Conduct individual, group, and family therapy sessions via telehealth platforms, ensuring that therapy is tailored to meet each patient’s unique needs and circumstances.

• Perform comprehensive psychosocial assessments to identify barriers to optimal medical care and develop customized treatment plans that prioritize patient well-being.

• Implement evidence-based interventions including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care techniques to address emotional and psychological challenges.

• Provide crisis intervention and case management services to patients in need, coordinating with other healthcare providers to ensure comprehensive care.

• Maintain accurate and confidential patient records within our electronic health record (EHR) system, ensuring compliance with all regulatory requirements and internal policies.

• Engage in regular clinical meetings, case reviews, and professional development opportunities to foster continuous improvement and collaboration among team members.

• Advocate for patients by connecting them to community resources and support services that aid in their overall health and recovery.

Patient Population / Client Focus\n

In this role, you will primarily work with patients receiving hospice care, which often includes individuals facing terminal illnesses and their families. This population may experience complex emotional, psychological, and social challenges related to end-of-life issues. Your ability to provide compassionate support and effective intervention will greatly enhance the patient and family experience during this critical time.

Qualifications\n

• Possession of an active LCSW license in California, in good standing.

• A Master’s degree in Social Work (MSW) from an accredited institution.

• A minimum of 2 years of post-licensure clinical experience, specifically in mental health or social work settings.

• Proficiency in evidence-based therapeutic approaches such as CBT, DBT, and trauma-focused interventions.

• Excellent communication, assessment, and counseling skills, with the ability to engage patients in a remote setting effectively.

Preferred Experience\n

Compensation and Benefits\n

The salary range for this position is $100,000 to $130,000 annually, commensurate with experience and qualifications.
