Licensed Practical Nurse - LTC

LTC/SNF LPNWe are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. The ideal candidate will be responsible for providing high-quality patient care to residents in a supportive environment.Key ResponsibilitiesDeliver patient care in accordance with established protocols and standards.Administer medications and treatments as prescribed by physicians.Monitor patient health and report any changes to the healthcare team.Assist with daily living activities, including bathing, grooming, and feeding.Communicate effectively with patients, families, and staff to ensure continuity of care.Participate in care planning and documentation using electronic medical records (EMR).Shift DetailsCombination of 8-hour and 12-hour shifts available.Day shifts: 6:00 AM - 2:00 PM / 6:00 AM - 6:00 PMEvening shifts: 2:00 PM - 10:00 PMEvery other weekend and holiday coverage required.QualificationsMinimum of 2 years of LPN experience in a long-term care or skilled nursing environment.BLS Certification required.Strong communication and interpersonal skills.Ability to work collaboratively within a healthcare team.Additional InformationLength of assignment: 13 weeks.Weekly guaranteed hours: 36.Overtime opportunities available.Orientation will include building familiarization and training on equipment.Common patient diagnoses include LTC residents with a focus on memory care and dementia.If you are committed to providing compassionate care and meet the qualifications listed above, we encourage you to apply for this rewarding opportunity.
