Connected Health Care
Nutrition Care Specialist
Provide inpatient and outpatient nutrition care for patients in Sandusky, OH. Develop individualized care plans, collaborate with medical staff, educate patients, and maintain documentation to support optimal health outcomes.
Key Responsibilities:
- Develop and implement individualized nutritional care plans for inpatient and outpatient clients
- Collaborate with medical staff to assess patient needs and recommend appropriate dietary adjustments
- Educate patients on nutrition and healthy eating habits tailored to their medical conditions
- Maintain accurate documentation of patient progress and dietary interventions
- Execute regular evaluations and modify nutritional plans to ensure optimal patient outcomes
Qualifications:
- Bachelor's degree in Dietetics, Nutrition, or related field
- Registered Dietitian (RD) certification
- Ohio State Dietitian License
- CPR certification
- Strong understanding of nutrition guidelines and patient care standards
- Excellent communication and interpersonal skills
- Ability to work independently in a hospital setting
- Flexibility to adapt to varying patient needs and schedules
Preferred Skills:
- Familiarity with inpatient and/or outpatient dietary services
- Clinical or outpatient experience
