Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments for a diverse patient population, ensuring culturally competent evaluations that address mental health, substance abuse, and associated medical needs.

• Engage in therapeutic interventions, including individual and group therapy, crisis intervention, intake assessments, and treatment planning tailored to the unique needs of patients and families.

• Maintain thorough and accurate electronic documentation of mental health evaluations, progress notes, and treatment updates, ensuring compliance with organizational and regulatory standards.

• Serve as a liaison, providing social work support and facilitating referrals to local community resources, including medication-assisted treatment programs and support groups.

• Collaborate with Quality Improvement teams to analyze and improve processes for patient care, ensuring exemplary standards of service delivery.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families coping with serious illnesses and end-of-life considerations within the hospice setting. This role requires a deep understanding of hospice care dynamics, addressing both psychological needs and emotional support during challenging transitions. Qualifications\n

• A valid and active state license as a Licensed Clinical Social Worker (LCSW) is required.

• A Master’s degree in Social Work is necessary, accompanied by three (3) years of post-licensure experience in clinical settings.
