Position Title: Program Coordinator - Partial Care Department: Partial Care FTE/Classification: Full-Time / Exempt Location: 125 Atlantic St, Hackensack, NJ 07601 Schedule: Monday - Friday (8:30am - 4:30pm; onsite) Salary Range: $65,000 - $75,000 (depending on years of experience and related experience.  

The Program Coordinator of Partial Care is a licensed clinician who assists the Director in general oversight and supervision of all aspects of the program, including clinical and administrative tasks. The Program Coordinator provides coverage for the Clinical Director when they are not present and coordinates the group counseling component of the program. Group Counselors report to the Program Coordinator. The Program Coordinator carries a reduced caseload and may conduct the functions of a counselor and senior counselor including caseload responsibilities, intake and member assignments and oversight of interns.

 

SUPERVISORY RESPONSIBILITIES:

1. Provides direct supervision to assigned employees to enhance their job skill set.
2. Provides on-going support to employees to assure completion of job responsibilities
3. Carries out supervisory responsibilities in accordance with the agency's policies, applicable laws and governmental regulatory agencies
4. Reviews employees' responsibilities to determine time frame and scope of authority, reviews employees' requirements and allotments of available resources
5. Evaluates employees' productivity and documentation of service delivery
6. Responsibilities include interviewing and training employees; planning, assigning and directing work, appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

PERFORMANCE RESPONSIBILITIES: Although each position has its own unique duties and responsibilities, the following list applies to every employee of North Jersey Friendship House:

6. Completes all required documentation/paperwork accurately and within established time frames.

Rehabilitation Counselor Responsibilities:

As a member of the counseling team the Program Coordinator may carry a reduced caseload and conduct all the functions of a counselor including:

1. Collaborates with individual clients to develop a Behavior Health Care Plan (BHCP) with treatment goals and strategies that are consistent with their current diagnosis, symptoms, and discharge planning criteria.
2. Ensures that discharge planning begins at intake.
3. Develops an individualized program schedule that includes daily groups and individual counseling. Referrals to groups are consistent with individualized treatment planning.
4. Completes the Biopsychosocial assessment at the appropriate intervals per regulation and NJFH policy.
5. Completes all Greenspace assessments per NJFH policy.
6. Consider pre-vocational training groups that are consistent with discharge planning goals and employment potential.
7. Work collaboratively with the members, other counselors, and rehabilitation aids to determine appropriateness, skill level, and treatment applicability for pre-vocational placement.
8. Maintain ongoing communication with outside agencies, members of the person's circle of support and medical professionals involved in member's care; documenting all contact and attempts to reach out, to ensure consistent direction of care.
9. Explore family dynamics and include family and other members of the person's circle of support in the treatment and discharge planning process.
10. Be familiar with the services of other health and welfare agencies and refer members for those services that will help maintain them in the community and improve their quality of life (i.e. Long-Term Care, Adult Day Care Centers, substance abuse services, ICMS, welfare, social security, mental health centers, outpatient care, respite care, recreational opportunities, etc.).
11. Complete timely reviews of the BHCP (per partial care regulations) Including realistic, achievable, and measurable goals that prioritize psychiatric rehabilitation and maintenance of gains aimed at discharge.
12. Complete a weekly progress note indicating the status of progress towards treatment planning goals.
13. Maintain continuous, precise case records in the EHR, following all DMHAS, CARF, and MCO requirements. Case records will provide a clear and current status of treatment progress, challenges and plans of intervention that have been developed in collaboration with the client.
14. Follow all current regulations for completing the BHCP and obtain signatures in a timely manner.
15. Complete all attendance and additional documentation, forms, and reports as required by NJFH
16. Attend all treatment team meetings and review cases with supervisor and/or in clinical meetings to address client needs, challenges, progress towards psychiatric rehabilitation and/or discharge.
17. Manage all crisis interventions, including psychiatric, medical, social, or behavioral situations. Provide immediate support; document actions in the EHR; follow up with members, caregivers, and medical personnel; plan for the member's return to the program; and complete incident reports as applicable.
18. Prioritize psychiatric rehabilitation and community integration in all cases. Engage members and staff in ways that support discharge planning from intake forward.
19. Develop a comprehensive discharge plan with measurable goals that includes follow-up services, including psychiatry, counseling, recreation, vocational development, and other services as needed.
20. Ensure that all relevant consents are obtained for communication with the circle of support including psychiatric practitioners, family/guardian and any other relevant service providers.

 

Psychosocial Group Program Supervisor - Essential Functions Include:

1. In collaboration with the Director of Clinical & Vocational Services, continuously review and revise the quality, content and strategies incorporated into psychoeducational and psychosocial group development and execution.
2. Ensure that group protocols are up to date, evidence based and relevant to members symptoms and treatment goals.
3. Periodically audit the quality and content of the group notes completed by all staff, including counselors and rehabilitation aids, and report findings to the Director.
4. Assign to counselors the revisions needed for group protocols and curriculum.
5. Coordinate with counselors the coverage for groups when needed as well as compliance with group size per regulation.

Additional Duties Include:

1. Attend and participate in all meetings as required by the Program Director
2. Coordinate and complete intakes including caseload assignment to counselor.
3. Interview and coordinate programs for interns and volunteers
4. Coordination of educational tours for prospective members, interns, students, etc.
5. Complete all training as assigned and per schedule.
6. Timely completion of any additional items requested by the Clinical Director or Chief Executive Officer.
7. In collaboration with the Program Director conduct Utilization Review Committee Meetings that address a minimum of 10% of the Partial Care Program caseload per quarter.

REQUIREMENTS:

• Master's degree in rehabilitation counseling, psychology, social work or in another human services field.
• Licensed in the State of NJ as an LPC, LCSW or Psychologist
• Must have a minimum of 3-5 years of experience in mental health, minimally 1-2 years in a supervisory role.
