Our smaller panel sizes, wide array of support services and virtual care options provide more flexibility for patients to engage in their own health care and with their team.DutiesProvide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examination, and patient education to Veterans in the Primary Care team setting.Works collaboratively with other health care professionals to accomplish patient, service, and medical center goals, promoting and embracing the Patient Aligned Care Team (PACT) model of care, utilizing Integrity, Commitment, Advocacy, Respect, Excellence (ICARE) values throughout the clinic.Distinguish between normal and abnormal findings to establish a diagnosis and initiate appropriate response to emergency health problems.Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral.Initiate appropriate referrals and consultations to other clinical services.Initiate, evaluate, and change orders for medications.Follow Performance Measure guidelines, including completion of clinical reminders; access and timeliness standards; prescription order entry, National VA formulary Requirements, and customer service standards.Demonstrates flexibility in responding to clinic demands, adhere to personal and professional obligations and responsibilities.Practice and maintain ethical standards, and sustain a work environment that is harmonious and customer service oriented.Assists with arranging continuity of care across the VHA system as needed.Works to provide leadership on site to ensure care is completed in required timeframes.Participates in Quality Improvement activities.Participates and attends medical center committees as assigned.VA offers a comprehensive total rewards package. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
