LPN

The Licensed Practical Nurse (LPN) provides compassionate nursing care to residents in a Rehabilitation and Extended Care Facility. The LPN collaborates with the interdisciplinary team including RNs, Physicians, CNAs, and Unit Managers to ensure residents' daily care needs are met. The role requires sound nursing judgment, effective communication, and the ability to manage clinical and administrative duties in a dynamic healthcare environment. Responsibilities:

• Provide compassionate care and treatment to residents with a customer service attitude.
• Collaborate effectively with the interdisciplinary team including Physicians, RNs, Unit Managers, CNAs, and Ward Clerks.
• Evaluate and observe changes in resident conditions using sound nursing judgment.
• Communicate promptly with RNs and Physicians regarding changes in resident status.
• Delegate assignments and supervise support staff to ensure appropriate care delivery.
• Order medications and supplies, prepare equipment, and assist with wound care as needed.
• Maintain a clean and hygienic environment for residents, including dining and personal hygiene areas.
• Schedule resident appointments and notify families about appointments and transportation arrangements.
• Input orders into computer systems, notify pharmacy, and maintain accurate documentation.
• Attend meetings as directed and participate in care meetings and discharge planning.
• Ensure compliance with policies related to controlled substances and documentation.
• Report building emergencies and incidents of suspected abuse, neglect, mistreatment, or exploitation promptly.

• High School diploma or equivalent.
• Completion of Licensed Practical Nurse course of study.
• Active and unencumbered New York State LPN license.
• At least 2 years of LPN experience.
• Strong communication and collaboration skills.
• Ability to work efficiently in a fast-paced, dynamic environment.
• Physical ability to lift 45 pounds or more, perform repeated bending, kneeling, and hand-eye coordination tasks.

• BLS/CPR certification preferred.

• Must maintain compliance with controlled substance policies and accurate documentation.
• Responsible for reporting any suspected abuse, neglect, or exploitation immediately.
• Work environment includes Rehabilitation and Extended Care Facility with patient care and administrative duties.

• Nights shift from 7:00 PM to 7:30 AM.
• Every other weekend and holiday work as per unit needs.
