JR 5546 Case Manager - Shallow Subsidy

Role Description The JR 5546 Case Manager – Shallow Subsidy is a full-time, on-site role based in the Los Angeles Metropolitan Area. This role focuses on supporting households receiving shallow rental subsidies to maintain stable housing and prevent a return to homelessness. Day-to-day responsibilities include conducting intake and assessment, developing individualized service plans, and coordinating housing stabilization strategies with clients and partners. The Case Manager provides ongoing case management, connects clients to benefits and community resources, and offers coaching around budgeting, tenancy, and life skills. The role also involves maintaining accurate documentation, tracking outcomes, collaborating with landlords and service providers, and adhering to program and compliance requirements. Qualifications Demonstrated experience in case management, social services, or housing-focused programs, preferably with people experiencing or at risk of homelessness. Knowledge of housing resources, public benefits, and community-based services within the Los Angeles Metropolitan Area. Strong skills in client engagement, motivational interviewing, crisis intervention, and trauma-informed, person-centered practices. Ability to manage a caseload, organize time effectively, and maintain accurate, timely documentation in electronic databases. Clear, professional written and verbal communication skills, including the ability to collaborate with landlords, partners, and multidisciplinary teams. Cultural humility and commitment to equity, inclusivity, and non-discriminatory service delivery. Proficiency with basic office software and data entry; comfort with learning new systems and tools. Relevant education or training in social work, psychology, human services, or a related field; equivalent experience considered.
