Work Environment

You will work in a dynamic onsite rehabilitation facility where teamwork and communication are essential. The facility is equipped with state-of-the-art therapy equipment and technology, allowing you to implement diverse treatment strategies tailored to each patient's needs. Our schedule runs Monday to Friday from 9 AM to 5 PM, offering a balanced work-life dynamic. Key Responsibilities

• Conduct thorough evaluations to assess patients' physical conditions, understanding their challenges and goals to design an effective treatment plan.
• Implement personalized therapy sessions focusing on enhancing mobility, relieving pain, and preventing further health decline, using hands-on techniques such as therapeutic exercises and balance training.
• Educate patients and their families about safe mobility practices and home exercise programs aimed at fostering independence.
• Collaborate closely with an interdisciplinary team, including nurses and other healthcare professionals, to coordinate comprehensive patient care.
• Maintain meticulous documentation of treatment plans, patient progress, and outcomes to ensure compliance with facility guidelines.
• Stay updated with the latest advancements in physical therapy practices and methodologies to continually enhance patient care.

Patient Population / Client Focus

Your work will primarily focus on patients recovering from surgery, trauma, or chronic conditions that affect their physical function. This includes geriatric patients in skilled nursing environments, requiring a compassionate understanding of their unique needs. Your robust skill set will be essential in assisting them to regain their independence and improve their quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program (B.S., M.S., D.P.T., or equivalent certification).
• Valid Physical Therapy license in the State of New York.
• Current BLS for Health Care Providers certification.
• A solid understanding of the Physical Therapy Practice Act of New York and adherence to the ethical standards set by the American Physical Therapy Association.

Preferred Experience

• A minimum of two years of experience as a Physical Therapist, preferably in skilled nursing, acute care, or outpatient settings.
• Direct experience in developing and leading rehabilitation programs for diverse patient populations.
• A track record of achieving positive patient outcomes in mobility and functional independence will be highly regarded.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000, supplemented by performance-based incentives.
