Work Environment

The position is located onsite at our fully equipped rehabilitation facility in Walden, New York, where teamwork and collaboration are at the heart of our practice. Our clinic operates Monday through Friday, from 9 AM to 5 PM, promoting a healthy work-life balance that allows you to focus on your professional responsibilities without the pressure of after-hours work.

Key Responsibilities

• Conduct thorough evaluations of each patient's condition to develop personalized treatment plans tailored to individual recovery goals.
• Implement hands-on rehabilitation techniques including therapeutic exercises, manual therapy, and modalities to enhance recovery.
• Monitor patient progress through regular assessments and adjust treatment strategies as necessary to ensure optimal outcomes.
• Educate patients and their families about recovery processes, self-management strategies, and home exercise programs to ensure continuity of care.
• Maintain accurate documentation of patient care and progress in compliance with organizational standards and state regulations.
• Collaborate with multidisciplinary teams to ensure cohesive care and effective communication amongst healthcare providers.

Qualifications

• Must possess a valid Physical Therapist license in the state of New York.
• Requirement of a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.

Preferred Experience

• Minimum of two years of clinical experience in an outpatient therapy setting.
