Nurse Practitioner Location: Takoma Park, MD Company: Carease Health - MD Summary We are seeking a highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team at Carease Health - MD. As a hybrid role, you will be responsible for delivering high‑quality preventative care to residents in skilled nursing facilities through a combination of on‑site visits and remote work from home. Our dedicated scribe team will provide support for efficient documentation. Responsibilities Provide preventative care to residents in skilled nursing facilities through on‑site visits and remote work from home Conduct evaluations, assessments, and treatment plans for patients Collaborate with healthcare teams to ensure comprehensive care and support Maintain accurate and timely documentation of patient records Participate in quality improvement initiatives to enhance patient care and outcomes Build strong relationships with facility staff and residents Requirements Active NP or PA license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Strong problem‑solving and critical‑thinking skills Ability to work independently and as part of a team Benefits Hybrid Role!
