Title Nurse Practitioner, Advanced Practice Provider Company Oak Street Health Location 7028 GA-85, Riverdale GA 30274 Schedule Full-Time, Monday through Friday, 8:00 AM - 5:00 PM Role Description The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on Medicare to keep them healthy and out of the hospital. Responsibilities Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments. Provide care coordination with other providers, specialists, testing facilities, and agencies. Lead population health initiatives, ensuring evidence-based screening requirements. Assist the care team with phone triage and outreach. Educate patients on health conditions, care plans, and treatments. Participate in Oak Street Health promotional activities and conduct home visits as needed. Perform other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one of: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner. Active, non-probationary state Nurse Practitioner license. Active DEA license. US work authorization. Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine. Knowledge of HCC documentation, ICD-10 coding, and HRAs. Passion for teamwork and cross-functional collaboration. Desire to be part of an innovative, evidence-based population health model. Bilingual proficiency in applicable areas. Pay Range The typical pay range: $87,035.00 - $187,460.00.
