Hybrid RN Case Manager Discharge & Care Coordination (PRN)

Havasu Regional Medical Center is seeking a Registered Nurse Case Manager for a hybrid role serving both inpatient and emergency department patients in Lake Havasu City, AZ. The position emphasizes discharge planning, care coordination, and resource referrals, with a focus on patient and family outcomes. The role requires an active Arizona (or Compact) RN license, ADN, and BLS, with ACM preferred.
