Work Environment

This full-time position is primarily onsite and focused on home care delivery, allowing you to work directly in patients' residences and engage with them in their everyday environments. Key Responsibilities

• Conduct thorough evaluations , reviewing patient history and performing specific assessments to identify needs and establish individualized treatment plans.
• Communicate evaluation findings and treatment plans effectively to patients and their families, ensuring that they are involved in goal-setting and feel empowered in their recovery journey.
• Implement and adapt treatment protocols tailored to the patient's unique presentation and ongoing progress, utilizing a variety of therapeutic techniques.
• Collaborate with interdisciplinary teams of healthcare professionals to ensure cohesive and comprehensive patient care delivery.
• Document all patient interactions accurately and in a timely manner, maintaining clear records that reflect the course of care, including evaluations, daily notes, and discharge summaries.
• Manage your schedule effectively , meeting productivity expectations while providing flexibility to accommodate patients' needs.
• Contribute to strategic departmental goals by participating in quality improvement initiatives and complying with regulatory standards.

Patient Population / Client Focus

This role primarily focuses on a diverse outpatient population, including individuals recovering from orthopedic surgeries, sports injuries, chronic pain conditions, and neurological disorders. You will be instrumental in helping these patients regain independence and enhance their overall quality of life through personalized rehabilitation plans that reflect their unique circumstances and goals.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current Physical Therapy license as required for Virginia.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong interpersonal skills and the ability to work effectively with patients and their families.
• Passion for delivering high-quality patient care and making a difference in the community.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy role, preferably in an outpatient or home health setting.
• Knowledge and experience with contemporary therapeutic modalities and rehabilitation technologies.

Compensation and Benefits
