Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.We are proud to be ranked #22 Best Places to Work in Massachusetts and #51 Best Places to Work in the United States!Position Details:Main Office location: Lawrence, MAService Area: Lawrence territory.Job Type: Full TimeSchedule: Monday - FridaySalary Range: $75,000 - $90,000Responsibilities:Initiates appropriate preventive and rehabilitative nursing procedures.Administers medications and treatments as prescribed by physicians.Conducts initial assessments of patients and families to determine home care needs.Performs thorough physical assessments and gathers comprehensive medical histories.Develops and revises individualized plans of care based on patient status and evolving needs.Collaborates closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.Responsively addresses inquiries and concerns from patients, caregivers, and medical teams.Provides counseling and support to patients and caregivers before, during, and after treatment.Supervises home health aides as required.Qualifications:Must hold an active Nursing license in the State where the position is based.Preferably 2+ years of nursing experience, with a focus on home health and geriatrics.Nursing degree from an accredited institution.Excellent bedside manners and exceptional communication skills.Working knowledge of healthcare laws and regulations.Maintain current BLS CPR certification.Possess a valid driver's license and own a reliable, insured vehicle.Our Benefits:Competitive Salary and Bonus Structure.Comprehensive benefits package including 401(k) match, HMO & PPO Plans, Dental, Vision, Life Insurance, Short-Term Disability, and Flexible Spending Account.Flexible schedule to promote work-life balance.Free gym access (Westwood Office).Free Snack Bar in the office!Pathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
