At Ellen Memorial Rehabilitation and Healthcare Center, we blend expert rehabilitation and skilled nursing with a warm, home-like environment for life-changing outcomes. As an LPN Licensed Practical Nurse, you will play a vital role in providing compassionate and high-quality care to our patients.

Responsibilities:

Participate in the delivery of quality patient care, under the supervision of the Registered Nurse, to a diverse population of patients
Administer medications, treatments, and therapies as ordered by the healthcare provider
Conduct vital signs, perform basic wound care, and assist with mobility and ambulation
Develop and implement individualized care plans, in collaboration with the interdisciplinary team
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Assist with daily living activities, such as bathing, dressing, and grooming, as needed
Participate in quality improvement initiatives to ensure optimal patient outcomes

Requirements:

Current LPN license in the state of Pennsylvania
Ability to lift, transfer, and reposition patients; walk and stand for long periods
