Family Health Navigator: Prenatal to Pediatrics Care
Erie Family Health Center
Erie Family Health Centers seeks a Birthing Families Navigator to coordinate prenatal, post-partum and pediatric care with medical teams, delivering supportive services to clients, conducting outreach, assessments and care plan referrals. Home visitation as needed and careful documentation in our funder data systems and EHR are essential.
Ideal candidates hold an Associate’s degree in social work or related field, with 2 years of relevant health or social services experience; bilingual
