Key Responsibilities\n

• Conduct thorough clinical assessments to identify the specific mental health needs of patients, facilitating personalized treatment planning.

• Administer evidence-based therapeutic interventions, including but not limited to CBT (Cognitive Behavioral Therapy) and mindfulness practices, while adjusting techniques to meet individual client needs.

• Maintain accurate and timely clinical documentation in our Electronic Health Records (EHR) system, ensuring compliance with state regulations and organizational policies.

• Collaborate with a diverse treatment team to integrate physical and mental health approaches, enhancing the overall treatment experience for patients and families.

• Engage in regular supervision and team meetings, sharing insights and strategies to optimize workflow and improve patient outcomes.

• Provide education and support to both patients and their families, equipping them with coping strategies and resources tailored to their unique circumstances.

• Stay informed about advancements in mental healthcare practices through ongoing training and professional development opportunities.

Patient Population / Client Focus\n

In this position, you will work primarily with patients within a hospice setting, supporting individuals and families during some of the most challenging times in their lives. You will help clients navigate a range of mental health concerns, including anxiety, depression, grief, and end-of-life issues, providing crucial emotional support and guidance as they confront these sensitive topics. Your role will be pivotal in fostering resilience and creating a safe space for patients to explore their feelings and experiences.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid Licensed Clinical Social Worker (LCSW) license in New York.
