Case Management Director

Pay Rate: $88,288.83 - $96,640.00 Status: Exempt Definition The Director of Case Management is responsible for providing strategic leadership and oversight for The Children's Cabinet's case management and care coordination services. The Director will lead the development and implementation of a consistent, family-centered case management model informed by the principles and practices of the National Wraparound Initiative. This position will work closely with program managers, supervisors, and other agency leaders to ensure high-quality, coordinated services that strengthen families, improve outcomes, and connect children and families to appropriate community-based resources. The Director will also lead the programmatic development and implementation necessary to transition eligible case management and care coordination services to Medicaid-reimbursable services. This includes developing service standards, workflows, documentation requirements, staff competencies, supervision structures, quality assurance processes, and other operational practices necessary to support Medicaid eligibility, compliance, and billing. Distinguishing Characteristics Broad understanding of case management, care coordination, family support, behavioral health, child and family systems, and community-based services. Demonstrated skills in program development, administration, supervision, quality improvement, and project management, with an emphasis on building staff and agency capacity related to high-quality case management and Medicaid-reimbursable services. This position serves as the agency's primary practice leader for case management and care coordination and is responsible for advancing consistent standards across programs while supporting the transition from traditional case management toward comprehensive, family-centered care coordination informed by Wraparound principles. Supervision Assigned managers, supervisors, coordinators, case management staff, and support staff Duties Understand and contribute to the agency's mission, strategic focus areas, goals, and objectives and support staff to do the same. Provide strategic leadership and oversight for agency case management and care coordination services. Promote coordinated service planning across agency programs to reduce duplication, improve internal referrals, and create a seamless experience for children and families. Work closely with Program Managers and/or Coordinators to ensure activities are aligned with agency expectations, service models, contracts, grants, and funding requirements. Lead the development, implementation, and maintenance of agency-wide case management practices, standards, policies, procedures, workflows, forms, assessments, service plans, documentation expectations, and tools informed by the National Wraparound Initiative and its principles, including family voice and choice, team-based planning, natural supports, collaboration, individualized planning, strengths-based practice, and measurable outcomes. Lead Medicaid readiness for eligible case management and care coordination services by developing clear service definitions, operational workflows, staff responsibilities, documentation expectations, supervision practices, quality assurance processes, and billing support practices. Translate Medicaid requirements into clear service definitions, workflows, staff responsibilities, documentation expectations, supervision practices, and quality assurance processes. Evaluate existing case management services and workflows to identify opportunities for Medicaid reimbursement and sustainable funding. Develop, monitor, and use quality assurance processes, case record and documentation reviews, program monitoring, caseload data, service delivery data, family engagement measures, referrals, successful connections to services, outcomes, and other key performance indicators to guide continuous quality improvement. Stay abreast of developments and best practices related to Wraparound, case management, care coordination, Medicaid, behavioral health, child and family services, and community-based systems of care. Work collaboratively with Finance, Compliance, Data & Evaluation, Information Technology, Human Resources, and program leadership to develop the infrastructure necessary to support Medicaid enrollment, documentation, service delivery, billing, and audit readiness. Partner with Finance and appropriate administrative staff to support processes related to Medicaid eligibility verification, service authorization, coding, billing, claim reconciliation, denials, and reimbursement. Work with Human Resources and program leadership to ensure staff qualifications, credentials, training, and supervision structures meet applicable service and payer requirements. Develop and maintain community partnerships with healthcare providers, behavioral health providers, schools, government agencies, managed care organizations, community organizations, and other partners to strengthen referral networks, service coordination, and access to resources for children and families. Foster communication and collaboration with healthcare providers, behavioral health providers, schools, government agencies, managed care organizations, community organizations, and other partners serving children and families. Represent The Children's Cabinet in community and statewide discussions related to case management, care coordination, systems of care, and Medicaid services as appropriate. Monitor assigned program budgets, staffing, contracts, resources, monthly program finances, adjusted journal entries, and year-end spending projections in coordination with Finance and the Executive Team as appropriate. Participate in program budget development and monitor assigned program budgets, staffing, contracts, and resources. Attend and participate in department and agency staff meetings and other events as needed. Set annual goals and objectives related to specific job functions and the Strategic Focus Areas. Serve as a member of the Leadership Team and mentor lead staff in advancing agency mission, strategic focus areas, goals, objectives, and assigned outcomes. Work with Human Resources to provide leadership and direction to assigned managers, supervisors, and coordinators, including professional development, performance planning and evaluation, hiring, and termination practices. Foster a collaborative and supportive work environment to promote team cohesion, accountability, continuous learning, and effectiveness. Performs other duties as assigned. Minimum Qualifications Knowledge of Leadership development, supervision, budget management, and nonprofit operations required. Case management, care coordination, family-centered practice, and community-based human services. Behavioral health, child and family services, and systems of care. Wraparound principles and comprehensive care coordination models. Medicaid-funded services, documentation, compliance, billing, and reimbursement practices. Electronic health records, case management systems, and documentation practices. Project and program management, including outcome measurement, tracking, reporting, and continuous quality improvement. Community needs, systems collaboration, and consensus-building. Equity and disparities in access to and utilization of support services. Community agencies and resources, as well as an understanding of the challenges facing children and families. Ability to Communicate using strong, concise written and verbal communication skills. Provide strategic leadership and guidance to staff in conjunction with agency goals. Translate complex Medicaid, regulatory, funding, and program requirements into practical workflows and staff expectations. Lead organizational and practice change. Develop managers, supervisors, and frontline staff through coaching, training, and performance management. Develop and maintain effective relationships within the agency as well as with community partners. Bachelor's degree in social work, human services, public health, psychology, counseling, healthcare administration, business administration, or related field, or at least 10 years of comparable experience in nonprofit/for-profit administration and human service delivery to diverse groups of people. Master's degree preferred. 10+ years of demonstrated experience in case management, care coordination, behavioral health, child and family services, human services, healthcare, or other related fields. Minimum 7 years of experience in supervision and management. Experience with Medicaid-funded services, healthcare reimbursement, managed care, behavioral health billing, or other third-party reimbursement strongly preferred. Experience implementing Wraparound, Systems of Care, intensive care coordination, or another evidence-informed case management model preferred. Must have reliable transportation for work and possess a valid Nevada Driver's License and proof of insurance for utilizing a personal vehicle for company business. May be required to work some evenings and weekends. May be required to travel in state and out of state for training. Must be able to lift and carry up to 25 pounds. Must be able to walk up and down stairs. Able to pass complete background check.

  1. Leadership: Program directors are expected to provide strong leadership to their teams, guiding them towards the successful completion of the program's objectives. This involves setting a clear vision, motivating team members, and resolving conflicts that may arise.
  2. Strategic Planning: Program Directors must have a strategic vision for the programs they oversee. This includes defining objectives, developing a roadmap for achieving them, and aligning program goals with the agency's overall strategy/mission. Develop clear program models and research. Directors must work with other program directors to ensure continuity across all program goals as well as collaborate with managers to identify gaps and need for continuous quality improvement.
  3. Communication: Program directors must possess excellent communication skills to effectively convey information to team members, stakeholders, and higher management. This includes written communication for reports, emails, and documentation, as well as verbal communication for meetings and presentations.
  4. Relationships: Offer training, mentorship, and support to help team members grow professionally. Recognize and celebrate the achievements and contributions of team members. Implement strategies to prevent conflicts and maintain a positive team dynamic.
  5. Risk Management: Program directors are responsible for identifying potential risks that may impact the program's success and developing mitigation strategies to address them. This includes monitoring risks throughout the program and implementing contingency plans when necessary.
  6. Budget Development: They must develop the program budget, ensuring that funds are allocated efficiently and effectively to meet project goals within budget constraints. This involves tracking expenses, forecasting costs, and making adjustments as needed.
  7. Quality Assurance: Program directors are accountable for the overall quality of deliverables and outcomes produced by the program. They must establish quality standards, monitor progress against these standards, and implement measures to address any deviations or deficiencies.
  8. Team Development: They are responsible for building and nurturing a high-performing team by providing guidance, support, and opportunities for professional development. This includes fostering a collaborative and inclusive work environment where team members feel motivated and valued.
  9. Adaptability: Program directors should be adaptable and able to manage change effectively, as programs often encounter unforeseen challenges or changes in requirements. They must be able to adjust plans and strategies accordingly to keep the program on track.
  10. Evaluation and Reporting: They need to conduct regular evaluations of the program's progress and performance against established goals and metrics. This involves generating reports and presentations to communicate program status, achievements, and areas for improvement to stakeholders and management.
  11. Partnerships: Identify and engage with stakeholders throughout the program lifecycle to ensure their needs and expectations are understood and addressed appropriately. They involve building and maintaining positive relationships with stakeholders from diverse backgrounds. Fostering strong partnerships with community partners can enhance the reach, effectiveness, and sustainability of their programs, ensuring that they meet community needs and achieve long-term positive impacts.
  12. Adherence to Mission and Values: Program Directors are responsible for ensuring that the programs align with the organization's mission, values, and strategic objectives. They should work towards fulfilling the organizations and communities overarching goals.
