Site: Mass General Brigham Home Care, Inc. Job Summary Mass General Brigham Home Care delivers non‑acute, medically necessary skilled care directly in patients’ homes to help homebound individuals recover from illness or injury and manage chronic conditions. Our interdisciplinary services include skilled nursing, physical therapy, occupational therapy, speech‑language pathology, medical social work, and medical supplies as needed. We are seeking a full‑time Speech‑Language Pathologist to join our dynamic Constitution team. Your time will be spent working in Arlington, Chelsea, Everett, Malden, Medford, Melrose, Revere, and Somerville. While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress‑free as possible. The typical schedule is Monday through Friday, with an anticipated caseload of approximately 4‑5 visits per day, plus time allotted for documentation, visit planning, chart review, and care coordination. Key Responsibilities Perform age‑appropriate evaluations that reflect patient history, relevant systems review, and appropriate tests and measures, and report findings to the primary clinician. Plan treatment and reassess at least every two weeks with the patient, primary caregivers, and interdisciplinary team; modify and communicate the plan as appropriate to meet discharge goals. Keep physician orders current and consistent with the plan of care and document them in the medical record. Submit patient schedules to the rehab scheduler by the end of day Thursday. Orient and supervise home health aides in accordance with agency and state guidelines. Manage patient caseloads in accordance with agency, state, and professional guidelines. Complete documentation and export it timely according to agency standards. Update POC devices weekly and as directed. Verify payor information on admission/evaluation within agency and regulatory guidelines. Maintain and transmit documentation to support Managed Care authorizations/DME requirements within designated time frames. Schedule visits in accordance with authorizations by Managed Care payors. Meet Agency Standards for casemix weight. Discuss utilization/caseload with the Rehab Manager on an ongoing basis and make changes toward stated goals. Ensure reportable outcomes meet Agency goals. Identify realistic, objective, time‑oriented goals for patient performance based on examination findings. Select treatment and discharge plans based on examination findings and patient goals. Inform patients of their weekly schedule and any other changes as necessary. Education / Degree Requirements Registered with the Board of Registration in Massachusetts. Licensure, Certifications or Registration Required: Registered with the Board of Registration in the State of Massachusetts. Experience Two years’ experience in acute care, rehab, or homecare as a Speech Therapist. Proficient computer skills with the ability to learn new software applications. Experience working in an interdisciplinary team. Strong verbal, written, and electronic communication skills. Excellent interpersonal and customer service skills with the ability to work with a diverse population. Who Thrives Here?
