Part-Time OUD MAT Liaison & Outreach Navigator
BayMark Health Services, Inc.
BayMark Programs is seeking a Part Time Patient Navigator to facilitate MAT linkage and outreach across EDs, jails, and related settings. You will identify eligible patients, complete MAT intake forms, and coordinate referrals, using strong communication and organizational skills. The role requires engagement with external entities, education on OUD and MAT, and support for insurance or financial counseling as needed.
