Summary The incumbent serves as the Point of Contact (POC) and Coordinator for the Military2VA (M2VA) Program at the Chalmers P Wylie Veterans Outpatient Clinic and assumes all of the technical and administrative responsibility for management of this program. The Transition and Care Management (TCM) Program Manager oversees all transition and coordination of care activities for Post 9/11 Servicemembers and Veterans (SM/Vs) treated at the facility. Responsibilities VA Careers - Licensed Clinical Social Worker: Total Rewards of a Allied Health Professional Duties and responsibilities include but are not limited to: Program Management Provides administrative oversight of the M2VA Program to include planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the functioning of the program. Follows Veterans Health Administration (VHA) policy requirements and develops local systems to identify and track post 9/11Servicemembers/Veterans (SM/Vs) served by the facility and community who belong in this special priority group. Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants. Ensures that the M2VA Program is interdisciplinary and integrated with the inpatient and outpatient medical programs. Establishes collaborative relationships with physicians, psychologists, nurses, social workers, and other disciplines to foster their involvement in the care of Veterans in the M2VA Program. Manages the daily operation of the facility M2VA Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction Oversight Of Case Management Services The M2VA Program Manager is responsible for and may supervise M2VA nurse and social worker case managers and facility Transition Patient Advocates. Ensures that all Post 9/11 SMNs are screened for the need for case management services and that those in need of nurse and/or social worker case managers are assigned accordingly particularly those who are severely injured or ill. Coordinates the assembly of the Inter-Disciplinary Care Management Review Team (IDCMRT) Meeting. Manages, coordinates, and advocates for the provision of individual and family treatment as well as other services as needed. Patient, Stakeholders, and Staff Education Provides consultation to other staff on the special needs of Post 9/11 SM/Vs. Provides education to Post 9/11 SMNs on VHA health care services available at the facility and about services available within the VISN and in the local community. Work Schedule: Monday Through Friday, 8:00 a.m. -4:30 p.m. Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Available, Ad- Hoc as determined by the agency policy. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship - Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). English Language Proficiency - Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education - Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. License - You must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates: Social Worker (Program Coordinator) GS-12 Experience Requirement: You must possess one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Examples of specialized experience at the GS-11 include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. Licensure Requirement: You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, you must demonstrate the following Knowledge, Skills and Abilities: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
