Community RN Career Direct Care Pathway Structure
The Community RN Career Direct Care Pathway consists of four distinct levels, determined by the RN's experience, education, and qualifications.
Level I - Less than 12 months of RN experience.
Level II - 12 months to less than 3 years of RN experience.
Level III - 3 years to less than 8 years of RN experience.
Level IV - 8 years or more of RN experience.
Position Summary
The RN Complex Care is responsible for delivering direct nursing care to patients and their families, adhering to established policies and procedures within a complex care environment.
Location: Main Campus
Unit: ICU-1
Shift: 36 hours, Evenings | Nights 7:00 PM - 7:00 AM
Weekends and holidays are required.
Unit Overview:
ICU-1 is a 12-bed Intensive Care Unit that caters to critically ill patients with various medical and surgical conditions, including cardiac, pulmonary, neurological, hematologic, obstetric, gastrointestinal, renal, vascular, and multi-system diagnoses. Patients may need hemodynamic or respiratory support, close monitoring, or intensive nursing care, which can include the use of IABP, Impella devices, lysis, EKOS, TPA, hemodialysis, CRRT, PA lines, or 24-hour EEG monitoring.
The ICU team benefits from ongoing training and educational opportunities, including AACN’s online training program ECCO for newly hired nurses. Our commitment is to provide exceptional patient care to every individual in the ICU.
Job Responsibilities
Minimum Qualifications:
- Valid RN licensure in Massachusetts.
- Current Basic Life Support (BLS) certification.
- Department-specific certifications relevant to the patient care provided may be necessary.
- Experience level based on the assigned category (I, II, III, or IV).
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
Duties and Responsibilities: The following duties outline the general nature of the role and are not exhaustive. Additional responsibilities may be assigned as needed.
- Engages in and maintains awareness of performance improvement initiatives.
- Evaluates and incorporates evidence-based practices and research into clinical care.
- Conducts and accurately documents clinical assessments.
- Creates, implements, and assesses individualized patient care plans.
- Utilizes professional nursing judgment to tailor care plans based on patient assessments.
- Manages patient assignments by delegating tasks effectively according to patient needs and team members' skills.
- Develops comprehensive educational plans for patients and families, utilizing appropriate resources and documenting per departmental standards.
- Respects patient and family rights by supporting informed decisions and advanced directives.
- Continuously evaluates and updates care plans based on changes in patient health status.
- Creates thorough discharge plans, incorporating relevant resources and referrals, including community services.
- Assists in the development of new staff members and facilitates their integration into the unit.
- Mentors department staff to promote professional growth through precepting.
- Participates in professional development by identifying educational needs and pursuing relevant training.
- Engages in departmental committees as required.
Physical Requirements:
- Occasional lifting of 30-35 lbs and repositioning of patients.
- Prolonged periods of standing and walking.
- Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
- Exposure to infectious diseases and potential carcinogenic substances.
- Exposure to blood-borne pathogens.
- Fine manual dexterity is required for patient handling.
- Visual and auditory acuity necessary for monitoring patient care.
Skills & Abilities:
- Strong communication skills with patients, families, physicians, and healthcare teams.
- Knowledge and application of the nursing process.
