RN - Lead Care Manager (Full-Time, Day Shift)

Role Overview: Provides exceptional patient care by collaborating on, developing, implementing, and reporting on the case management program. Acts as a liaison among patients, families, healthcare providers, and multidisciplinary teams to optimize outcomes and ensure coordinated care. Serves as a consultant on patient-specific issues for the healthcare team. Key Responsibilities: Leads patient care coordination, monitors the appropriateness and timeliness of care, and ensures care plans align with clinical needs and coverage. Collaborates with physicians on resource utilization, treatment plans, and discharge planning, especially for complex cases, frequent ED visitors, and patients with chronic conditions. Manages patient transfers to tertiary centers and ensures compliance with regulatory standards. Handles data collection for accreditation and manages frequent ED utilizers through focused reviews. Qualifications & Skills: Requires a valid RN licensure and a Bachelor's Degree in Nursing (preferred). Experience in care management and case management certification are advantageous.
