Physical Therapist (PT) - Home Health

We are seeking a dedicated Physical Therapist (PT) to provide exceptional home health care to patients in the Mt. Angel, OR area and surrounding regions. The ideal candidate will possess a strong background in skilled home health rehabilitation.

Key Responsibilities

• Conduct one-on-one patient evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to promote patient mobility and improve physical function.
• Document patient progress and adjust treatment plans as necessary using EPIC Remote Client for Home Health and Rover/Oasis.
• Minimize drive time through effective client grouping strategies.
• Ensure compliance with all safety protocols and organizational policies.

Qualifications

• Minimum of 1 year of skilled home health experience required.
• BCLS certification is required.
• Oasis experience is necessary.
• Must have own transportation for patient visits.
• This position accepts candidates with a pending license.

Work Schedule

The position offers 8-hour day shifts, and the traveler may visit up to 6 patients per day while maintaining a typical productivity standard of 30 points. This role does not require weekend shifts or floating.

Special Requirements

• COVID vaccination primary series plus one booster is mandatory.
• Guaranteed hours are provided with the flexibility for the facility to call off up to one shift per pay period.
• Travel radius may extend up to 60 miles, with an expected total daily drive time of 60-90 minutes.

Business casual attire is encouraged, with scrubs allowed but not required. Free parking is available, and mileage reimbursement is provided at the IRS standard rate.
