Case Manager - Social Work

LiveWell Homecare Agency, LLC

Conduct psychosocial assessments for inpatient members Develop care and discharge plans tailored to patient needs Coordinate services across medical, behavioral health, and community resources Collaborate with healthcare professionals to facilitate safe transitions of care Support patient and family understanding of treatment plans Reduce readmissions through effective care management Ensure accurate documentation and proficiency with EHR systems Commit to continuous improvement and patient-centered care Qualifications & Skills: Strong documentation and EHR proficiency Excellent collaboration and communication skills Ability to develop comprehensive care plans Commitment to patient-centered, quality care #J-18808-Ljbffr LiveWell Homecare Agency, LLC
