Intake CoordinatorWe are seeking a warm, organized, and detail-oriented Spanish-speaking Intake Coordinator to join a care management program serving Long Island. In this role, you'll help individuals and families navigate services, insurance requirements, referrals, and enrollment.During your initial months, you'll also serve as a Care Manager, conducting monthly home visits and providing ongoing support to members and families.ResponsibilitiesCoordinate the intake process, including reviewing referrals, assessing eligibility, and supporting enrollmentConduct monthly home visits and provide care management supportWork with families, healthcare providers, therapists, community organizations, and Medicaid Managed Care PlansConduct assessments and develop individualized care plansMaintain accurate, timely documentationFollow applicable New York State Department of Health, Health Home, and regulatory requirementsAttend weekly virtual trainings, team meetings, and program updatesHelp maintain a current directory of services, providers, and community resources to support appropriate referralsQualificationsBachelor's degree requiredSpanish bilingual skills requiredStrong organizational, communication, and computer skillsDetail-oriented, dependable, and able to work independently and as part of a teamComfortable communicating with families and working through paperworkStrong problem-solving skills and a proactive approachExperience in healthcare, social services, psychology, case management, government programs, or insurance is a plusWhat We OfferFull benefits packageGenerous paid holidaysSupportive team and training on systems and requirementsOpportunities for growthMeaningful work helping individuals and families access servicesLocation: Hempstead, New York
