Community Health Navigator & Care Coordinator

Lāna‘i Community Health Center

The Lāna‘i Community Health Center seeks a Community Health Worker to assist providers and staff in delivering comprehensive health care. The role focuses on helping patients navigate services, connecting families with resources, and promoting healthy behaviors through outreach and home visits. The CHW will perform health screenings, document interactions in the EHR within 24 hours, and support transportation and education efforts. Certification as CHW and bilingual skills are pluses. #J-18808-Ljbffr
