Care Coordinator II

Counseling + Recovery Services

Minimum Qualifications Bachelor’s/Master’s degree in a behavioral health related field or a Bachelor’s/Master’s degree in Education with at least 9 hours of credit in a behavioral health related field. Minimum of one year work experience with children in the mental health field or related social service field. Requires the willingness and ability to work irregular hours as needed to ensure quality standards are met in service delivery. Special Knowledge, Skills and Abilities Ability to establish working relationships with clinical, administrative, and support staff as well as community partners to develop professionalism and coordination within the team. Must possess good working knowledge of community social service resources, basic crisis management skills, behavioral health resources, ability to provide trauma‑informed care and a commitment to the Sanctuary Model components. Ability to communicate clearly and in an emotionally intelligent manner with clear, polite, and respectful communication with staff, clients, and the community-in person, through written correspondence and on the telephone. Must always demonstrate professional and ethical behavior as well as the ability to maintain confidentiality guidelines for interactions regarding client information. Ability to work well under pressure, meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements. Certification in mental health services or related field may be required in order to provide services, i.e. Case Management Certification, Wellness Coach Certification. Basic computer knowledge and ability to use Microsoft software such as Outlook, Excel, and Word. Understands and promotes Trauma‑Informed Care practices. Practice and promote the Sanctuary Model’s seven commitments of non‑violence, emotional intelligence, social learning, open communication, social responsibility, democracy, and growth/change. Position Responsibilities To provide targeted support services (behavior management, daily living skills, social skills); linkage with community supports; referral to local needed services; advocacy; service coordination; and follow‑up on behalf of client and their family who are experiencing emotional and/or behavioral challenges that are significantly interfering with their success at home, school, work or in the community. A, B, C, D Manage a crisis involving a client, including a threat of suicide or harm to others. A, C, D Lead non‑diagnostic interview with the client and his/her family to assess service needs. A, C, D Provide services to clients in their homes, communities, and schools (if applicable) while incorporating the entire family and supports in the treatment program, with a focus on their strengths as well as the client’s age, developmental level, and cultural heritage. A, B, C Maintain close communication with the supervisor and other clinical and administrative staffs in order to assure the highest quality of services are delivered to the client. B, C Responsible for teaching about and implementing regular community meetings and safety plans with client(s) as part of commitment to the Sanctuary Model of client care. A, C Utilize appropriate de‑escalation techniques with clients who are agitated, anxious, angry, or in other vulnerable emotional states. B, C Provide and/or participate in ongoing Inservice training, as needed. A, B, C To provide services consistent with the Sanctuary Model assuring that care is client‑centered, trauma‑informed with the needs of the client and/or family directing the types of services provided. A, B, C Provides outreach services as necessary to clients when they appear to disengage from the service. Uses a variety of methods to provide outreach. C Meet established productivity expectations, when applicable. A, B, C, D Maintain current clinical case records and other required documentation in a manner to meet established requirements. A, C, D Have knowledge of psychotropic medications and their actions. A, C, D Attend weekly staff meetings consistently as well as other departmental, clinical, or agency meetings when scheduled. E Coordinating services between agencies and developing cooperative working relationships with agencies serving children and families. A, B, C, D Children Specific Services Provide targeted support services within a system of care that is client‑centered and trauma‑sensitive with the needs of the client dictating the types services provided. A, B, C, D For Wraparound Clients: Complete initial care plans, strengths and cultural discoveries and intakes as needed. Update and modify care plans as needed. C, D, E To assist in preventing an out‑of‑home (or school) placement and/or in facilitating discharge from an inpatient or residential treatment facility. A, B, C, D Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs; Complete flex‑fund requests when necessary. D, E Contact referred families and the referral source for screening and notify providers of care team meeting dates. D, E Decrease of inpatient stay through alternative service planning and coordination of follow‑up referrals. A, B, D Provide targeted and continuous follow‑up to appropriate, least restrictive and least intrusive services. A, C, D Provide basic social skills training in such areas as: anger management, feelings identification, positive interpersonal communication, impulse/self‑control, healthy relationships, and other related coping skills, using a best‑practice curriculum. A, C, D Crisis Diversion Specific Services Will work as a part of the Crisis Diversion Team to provide immediate intervention for individuals admitted to CRS CCBHC who are at immediate risk of admission to local hospital emergency departments, psychiatric hospital inpatient units, and jails. A, B, C, D Provide immediate crisis interventions whether on phone or in person; including a threat of suicide or harm to others; utilizing skills/interventions to de‑escalate individuals who are agitated, anxious, angry, or in other vulnerable emotional states. A, C, D During daytime working hours (while main office is open), the crisis diversion CM will be responsible for answering Daytime crisis phone and responding to crisis situations within the office (i.e. med clinic, lobby, etc…). A, C, D Collaboratively develop emergency safety plan/safety promise with individual and/or family that ensures safety, matches the individual’s competency and skill level, and incorporates the person’s strengths and supports. A, C, D Responsible for follow up on clients who screen into suicide protocol after initial evaluation appointment. This will consist of a 48 hour follow up contact with the client and the 2‑week reassessment of PHQ-9, Columbia, and safety plan. D, E Participate in verbal de‑escalation and interventions in emergent situations and be willing to assist other team members as needed to maintain a safe and secure environment. D, E Will be available to assist any walk‑in with a crisis screening and in the moment safety plan to reduce risk of hospitalization. This may look like exploration of coping skills, social supports, education on triggers and mental illness/substance abuse (expansion of crisis walk‑in hours to all day during business hours). A, C, D Responsible for hospital/general PCP referrals faxed into the agency for psychiatric/psychological issues. A, C, D Responsible for calling client and screening for services as well as setting appointment for walk‑in or initial evaluation with intake specialist. A, C, D Partners with each client to provide information about service options and the process of care. A, C, D Overnight/weekend duties will include answering the afterhours crisis calls via phone or iPad. This will include completion of crisis diversion form and all required documentation within EHR. Will also follow up with the individual in crisis within 24 hours of call. Will staff crisis cases with supervisor and appropriate team upon office opening next business day. CDC 21 information completed. C, D, E Prepare and send letters for follow up care including, but not limited to, missed appointments, expired plans, and discharged clients. C, D, E Safety‑Sensitive Counseling & Recovery Services of Oklahoma has designated this position as “safety‑sensitive.” Employees who work in positions classified as “safety‑sensitive” can be disciplined if they test positive for marijuana or its metabolites. Possession or use of medical marijuana while at work or during the hours of employment regardless of whether the employee has a valid Oklahoma medical marijuana license, is still strictly prohibited. Other Duties Perform other duties as assigned. Physical Requirements Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data into various programs. Information shall only be accessed on a need‑to‑know basis, in the scope of the job. Employment Status Non‑Exempt Supervises None Hours Irregular hours including some evening and/or weekend work may be required as needed to ensure quality standards are met in service delivery. This position has a weekly productivity requirement of 22.5 billable services each week. Documentation must be entered into the EHR within 48 hours of providing the service per agency standards.
