About the Position

Job Title: Home Health LMSW Profession: Social Services Specialty: LMSW (Licensed Masters of Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 1 year of skilled home health experience License: OR licensure required Certifications: BCLS required Must-Have: Oasis experience required Description: Home Health LMSW position available. The candidate must have experience in home health care. The position includes visiting up to 6 patients per day, with a standard productivity point requirement. No weekend work is required. The traveler is expected to have their own transportation. The facility does a good job of grouping clients to minimize drive time. The maximum driving radius is approximately 60 miles, round trip. The facility requires the primary series of COVID vaccination, along with one booster. The position requires the ability to document care accurately according to the plan. The traveler may need to work with cultural issues and engage with other health care programs. Education on advance directives and care coordination is expected. The traveler is expected to participate in care conferences and develop a plan of care to meet patient and caregiver needs. All candidates must adhere to the schedule and conditions set forth by the facility.
