Work Environment

This is an onsite position operated from our Home Care facility, where you will work closely with a team of healthcare professionals in a supportive environment. Our team collaborates not only to address immediate health issues but also to develop long-term wellness strategies tailored to individual patient needs. The typical work week is structured, allowing for balanced hours from Monday to Friday, with no required weekends or on-call duties.

Key Responsibilities

• Perform thorough primary care assessments and evaluations to develop effective treatment plans tailored to each patient.
• Conduct onsite health risk assessments to identify potential health issues and collaboratively devise strategic intervention plans.
• Deliver patient education on wellness, preventive measures, and promote healthy lifestyle choices that empower patients.
• Facilitate lifestyle transformation by supporting patients in making informed changes to improve their health.
• Assist patients with chronic disease management through evidence-based care plans and continuous monitoring.
• Oversee and coordinate the medical staff's care delivery at the Marathon Health Center, ensuring a cohesive approach to patient health.
• Develop collaborative relationships within the healthcare team to enhance the patient experience.
• Engage actively in community wellness events, promoting education and health awareness.

Patient Population / Client Focus

Your focus will be on treating members of CEBT, providing them with comprehensive care designed to enhance their overall health and well-being. Our patient demographic is diverse, encompassing a range of ages and health backgrounds, representing the Glen Burnie community's unique needs.

Qualifications

• A Master's degree in Nursing with a specialization as a Family Nurse Practitioner.
• A minimum of 1 year of experience as a Family Nurse Practitioner, preferably within a primary care context.
• Current and unrestricted state licensure to practice as a Nurse Practitioner.
• Strong communication skills with the ability to foster trusting relationships with patients and colleagues.
• Commitment to ongoing professional development, including attendance at wellness fairs and events.

Preferred Experience

• Experience with Electronic Medical Records (EMR) is preferred to streamline patient care processes.
• Background in delivering health education and engaging with community health initiatives will be advantageous.

Compensation and Benefits
